2024-25 REP Tryout Registration Form
Please complete the form and submit if you plan on attending any division tryouts for Delhi Minor Hockey Rep Teams. Cost is $40 for 3 tryout skates per division. If you have registered in Delhi and have selected the Rep Registration, this would have been included in that price. If you haven't registered in Delhi (Ex. 3.5 from Norwich or NRP from other centres), you will have to e-transfer the fee to [email protected] Please use in the "Message Line" - Players Name & Reason for payment (ex. U11 Rep Tryout) Password "hockey" If you are an NRP (U12 and above) from another centre (ex. Norfolk, South Oxford, Brantford, etc.. ), ask your home association for an nrp request to be sent to [email protected]. Tryouts will take place in the first two weeks of September. Keep an eye on the organizations calendar!

TRYOUT AKNOWLEDGEMENT
I acknowledge and understand that the tryout fee of $40.00 is non-refundable. This is included in the "Rep Registration" for DDMHA players.  Coaching staff reserve the right to release players after two tryout sessions or anytime thereafter should additional tryouts be required. 

Players on a 3.5, ATM or NRP Form, need to be complete, and if they do not have their form the player will not be permitted on the ice.

Non DDMHA Fees must be paid prior to the first tryout and will be accepeted by e-transfer only. Please remit fees to the following email address: [email protected] with the following:
  1. Please use in the "Message Line" - Players Name & Reason for payment (tryout, registrations, development)
  2. Password "hockey"
