I acknowledge that this tournament is a representative team tournament and the team I confirm the team I am registering is not a select team or tournament team. 

(Please note that a position is only guaranteed once payment is received and cleared in full. Submission of registration does not confirm a spot in the tournament)

By checking the "acknowledge button" on this application form, the team manager or coach, on behalf of his team, releases the sponsors of the tournament, it’s officials, arena management and all concerned with this event, from any and all liability or any injury or accident which may be incurred by any player or team official while participating in the tournament or while travelling to and from this event and accepts all decisions of the tournament committee as final. All team officials hereby agree to abide with all tournament rules, regulations and decisions and that all registration information and papers are correct.

The team’s Minor Hockey Association must grant permission before entry can be accepted. Permission of the Association President is required. 

A OMHA Travel Permit must be completed, and forwarded with registration form to DMHA Tournament Convenor.  A copy with completed Official Roster must be on file with DMHA Tournament Convenor, before team is eligible to compete in tournament

Minor Hockey Association is granted for the above team to compete in the DMHA Tournament.
